Volunteer Form
Our background check process for volunteers uses a convenient digital form.
- Complete the electronic background check form and upload a valid driver's license.
- Allow 1-2 weeks for processing, after which your school will be notified of the results.
- Only one application is needed per school year and covers all events, students and school buildings.
- We no longer accept paper copies of background checks. If you are unable to complete the electronic form, please visit your school or contact Holly Roelofs at the administrative office for assistance at 616-254-6599.
