Annual Health Form
All students, regardless if they have a healthcare plan or require medication, must have an updated PowerSchool Annual Health Form completed for each school year. It is important to keep up-to-date health information as your student progresses. The form can be found under “Forms'' on your PowerSchool page.

Guidelines for Medication at School

It’s preferred that students take their medications at home; however, we realize this is not always possible. Please review the following guidelines regarding student use of medication during the school day. As a reminder, the district does not stock any over-the-counter medication for student use.
- Medication Administration Consent Forms are accessible on the district website and available at your child’s school.
- Medication Administration Consent Form must be completed annually for each medication brought in for student use. Multiple medications cannot be listed on the same form.
- Medication Administration Consent Form for prescription or non-FDA approved medications must be signed by a parent/guardian AND physician.
- Medication Administration Consent Form for FDA approved non-prescription medications require a parent/guardian signature only, if given as directed. Any dosing outside the recommended amount requires a physician signature.
- All medications must be unexpired and brought to the school by a parent/guardian. Medication cannot be sent in with a student.
- Medications must be in their original container with an accurate pharmacy or factory label attached. No medications will be accepted in a bag, envelope or other unmarked container.
- Emergency Medications such as an inhaler, insulin, epinephrine auto injector, or seizure medication may be carried by students who have received signed permission from their physician AND parent.
- High School students are allowed to carry their own medication, if it is deemed necessary by the physician and/or parent. The medication must be in the original container and limited to one day’s worth of doses. Students will be required to meet with their school nurse prior to carrying medication to review Grandville Public School’s self-carry procedure.
- If students have an Asthma Management Plan, Seizure Action Plan, Anaphylactic Allergy Plan or Diabetic Forms from a physician, all physician orders and instructions should be written on the plan, signed by parents and physician, and sent to the medical fax number (616) 254-6227. Please contact your school nurse directly to create/review your students' School Healthcare Plan each year.
If your student has an Individualized Healthcare Plan for a critical health need, supplies specific to their condition are the responsibility of the parent/guardian.

Seventh-Grade Immunizations
According to state law, all students entering seventh grade in a public school in Michigan need to have their immunizations reviewed and updated if needed. In addition to the vaccines required at kindergarten entry, there are two immunizations specifically required for seventh grade: Tdap and Meningitis vaccines. All students entering seventh grade will need to have their immunizations updated before the first day of school.
If there are philosophical or religious reasons to waive the school-required vaccines, contact the local Health Department to obtain a certified waiver. For children with medical conditions prohibiting the administration of any of the school-required vaccines, a State of Michigan Medical Contraindication Waiver form from your healthcare provider will need to be submitted.
