Parent Forms for Return to School

Dear Parents,
 
As part of our return to learning plan, parents must sign our Parent Acknowledgement and Verification Form in order for their child to return to in-person learning. The form provides the District assurances that the student does not currently have COVID-19 symptoms, has not been exposed to someone who is known to have COVID-19, and verifies that the student has not traveled in the past 14 days. Please print and sign the form then return the form via email, US Mail, or in-person to your building.
 
The Mask Waiver is also attached to this message. This is a required document for any student who is medically unable to wear a mask or face covering in school. This form must be completed and signed by your primary care physician. In addition, the doctor must provide a letter documenting the medical issue and detailing reasons why the mask or face covering impacts the student's medical issue. This form must be delivered to the school building either by email, US Mail, or in-person.
 
The third form is our typical medical update form that is sent home each year (it is typically pink). Please complete this form, sign it, and return it to your building by email, US Mail, or in-person.
